duplicate rows in database

while i am inserting one row in mysql table by php, the two rows inserted (duplicate) in table. any body tell me why it happening? and how to handle this problem.

i solved this problem. now i have another problem.
there are many records in table i want to display only one at a time and maximun ten records. limit function is used to display one but how can i display ten records, any body explain me with php code.

The clause would be: LIMIT 0, 10
