Answer Questions for a Certificate.

I need to create a php script which will have 5 questions. And based on the answers to the 5 questions, the script will create a certificate for them to print out for an immediate discount that they can bring into our office. I have been searching for 2 days across the web and have not found a viable solution or even something close that I can rework for the specific needs.

1) Monthly Income $0000.00
2) Have you been with your current employer for at least 6 months? YES/NO
3) In the last 3 years have you ever had any of the following?
a) (Yes / No Question 1) YES/NO
b) (Yes / No Question 2) YES/NO
c) (Yes / No Question 3) YES/NO

Question 1 will be divided by 3 and put on the certificate if question 2, 3a,b and c are all yes, if there is one no, then there will be put some default text on the certificate for them to give us a call.

The whole script and certificate can be on 1 page with the questions on top and the certificate on the bottom.

I don't know for certain if this is possible?

This will take about 5 minutes to code the decision making process of whether to print the 1/3 salary or the default text.

If you want the certificate to have a nicely formatted appearance, it should probably be done with a program like fpdf (which generates PDF files) or ImageMagick which generates image files. This would give you tight control over the specific look of the certificate. It will probably take you a month or two to learn either of these two programs.

Unless you don't care how the certificate looks. If appearance is unimportant, you can just construct a little HTML to make the certificate but when it gets printed, you lose some control of how nice it looks when it gets printed out. This is probably 15 minutes work for someone with PHP experience.

Your best bet is to hire a programmer and decide whether you want the quick version (1 hour or less) or the really nice looking certificate (5+ hours of coding).

Of course, you'll want to make that HTML look nicer but this gets you 3/4 of the way there.
