Need Help with am upload PHP code

I'm running Wordpress 2.8

I would like a script that lets the customer upload a file (jpg,png,gif,etc) pretty much an image file.
Then I would like that file to

(a) be uploaded to my server
(b) a copy sent via e-mail to me

any help?? I know there are some geniuses out there!

This will get you going uploading images

After that your best bet is to email yourself a link to the image vs. the actual image in the event its too big to be emailed

i actually tried google and that's what brought me here.

My questions are

do i just upload the script to my server or do I post it on the page I'm editing? or what?
