Problems with basic statements loading

Just starting out in PHP

Have installed all the necessary components in PHP, Apache and MySQL and nothing wrong with the install as far as I can tell (few .dll's missing for some PHP libraries) but when I try and load a simple PHP form even the echo statement doesn't load, any thoughts?

It's not my code came straight off a well known book PHP and MySQL Developers Library by Welling, so I am not sure where I might have slipped up in the install of the above three, any thoughts appreciated.

downloaded this proram and in installed fine but the form entry still doesn't work.

once it hits the first statement it goes haywire and doesn't work. just hits the php code and won't execute the action:form.

any further thoughts?
