Database Pull Way out of my element

Hi Everyone,

I'm am far from a Developer and need some help. I have a client that I designed a website for who now wants to create a forms page on the site which will go into a database.

Here's the background
Lets say a web designer fills a form out with their information and background, this is basically a job site.
Then you have a person looking for a designer and they are able to fill out a questionnaire which will then give them a list of designers with their specifications. Is this a PHP thing and does anyone know what sort of database system I need to create with this?

This is a request for code to be done, check out the freelancers part of the site - most of us here can write this kind of thing quite easily, but it does take planning and time investments that most of us are used to being paid for.
It's also too complex for a beginner to put together reliably, which means it's not something you can be taught to write overnight, you really need to work up to it a little.
