Uploading files in database

Need help!...I wanted to create a simple document management system and i want a sample codes for uploading text files .pdf files,/ .doc files and etc.. directly to the database..and also viewing all files that already uploaded and ready for download...

search the web for php upload file and you'll get plenty of tutorials on how to deal with file upload.
