PHP form data print as a customer report

PHP form data transferring to database (MySQL) is ok for me , but i need to print entered form data in to a A4 as a receipt for a customer in any format, if pdf is much better. i am working in ubuntu (Linux) OS. pls help me with codes how to do it. also i did with some exercises with fpdf but not successful.

if i were you, i were generate an image by GD library from the entered values.

Perhaps PDFLib would be more suitable (see [man]PDF[/man]).
