I need help creating two mailer forms that will be placed on two different web pages.
1st form as follows
(form name request)

Last Name
Company Name
Phone
Fax
Address
Address2
City
State
Country
e-mail
(Dropdown required) Name - (text field) Description - Quantity - Destination Country- Destination Port.

FORM#2
(Form Name Offer)
Same as above form except fields should be as follows.

(Image attachment) Attach Image Of Product - (Check Box) FOB, CIF (text box) location- Time Frame (month from and to)
