I would like my phpBB Users to fill out a application form that'll then post the information that they have filled out in a new post called [PENDING] <Their Name>

How could i or someone do this?

If someone is willing to do it for me and tell me how to install it, then please do! Here are the options i want.

[note: this is different to the registeration form, i'll have this form as the "Join Our Group", new members fill it out, and they await acceptance to my clan.]
