Logic: Load form to db, query record, send email

I need to write a function/script that does the following.

1) Loads form data from a web based php form into mysql db.
2) Immediately pull the email address from the db record and email back to user
3) Email body has a link to a secure website
4) User selects link and enters their email and zip code to access website

I know how to create the form that creates a new record in the db, but not how to do the rest. If you can point me in the right direction that would be most helpful.

Those should get you started there is also tons of tutorial on using a mysql database as well as tutorials on sending emails form php.

I have looked at the db and mail tutorials but cannot make the jump from A - D on this one.

Here is my problem, thanks in advance for help with this.

User fills out online form.

1. name, cell phone num, cell phone carrier, zipcode
2. upon submittal to db a script does the following
   2a. pulls cell phone and carrier id
   2b. formats and email address based on cell phone number with @carriertype
   2c. loads address into email message with link to webpage for user login
3. sends email to mobile carrier sms gateway

I just have not been able to fill this sequence in, I have tried mashing various code together but not working.
