i have a page where i input game stats. that page loads fine but when i click submit/add game it displays

any ideas on what might cause this?

Yep; code that outputs a '0' followed by code that outputs a ':', or code that outputs '0:'.

You might want to do a "view source" on the browser page when you get that result and see if there is any useful clues in the HTML source. (No promises, but sometimes useful things are output that get treated as an HTML tag or comment for whatever formatting reason.)
