Email form help

I am using a free email form that looks like below:

I would basically the last name field to accept all characters possible as it is a usernaqme field.
What revisions do i need to make?

Sorry, I am completely clueless when it comes to this.

be very careful using code you don't understand, it could open your system up to attack
