I created am email me form on emailmeform. Then I bought the code. And I can't make it work on the hosting I have and I know it's something in the code I'm missing. Can you please help me with this? When somebody fills in the form on my website I don't want to use the emailmeform.com I want to use the mail.php script hosted on my website to send the results in my e-mail.

Attached you have the php file and the html file. If you can help it would be really appreciated
