text based captcha (without images-GD)

Hi,
How To Create text based captcha with random question?
Example :
What is pal system ? answer : . . . . .
What is NTSC system ? answer : . . . . .
What is 3d system ? answer : . . . . .
What is LED system ? answer : . . . . .
What is LCD system ? answer : . . . . .
and etc . . .

this question add to any php file or mysql database! so for each question add 5 answer in <option></option> . user choose answer and submit . than php (or mysql) checked answer true and false and show resualt ( you Human - You not A human ).

Php Files : register.php - check.php( if php based question or mysql based ) - captcha.php
