I am creating a sign up page and when i fill out the form and submit i get this info... any advice?

It means your code failed to find and then include the connect_to_mysql.php file, and then presumably because your mysql_connect() was therefore not executed, you could not then use the mysql_real_escape_string() function. So you need to check the include_once() call and ensure that the file is where you are trying to access it. The later failure would then imply that you probably should be using require_once() instead of include_once(), since it's apparent that that file should be required, not optional.
