help with recruitment form

Hello all.
As my first post, I thought best place to post si the newbie forum!
We as a gaming-clan work with recruitment, which works fine; People fill in their info and by submitting it is sent to different people who deal with the recruitment.

Now we want to expand to a multi-game group, and therefor change our recruitment process. We still want people to fill the form in, but we want them to select the game(s) they play (multiple choice with more answers possible).
I want to have every game (choice) linked to different email addresses. If they choose game 'A' I want it to be sent to email '1' and when they choose 'A' and 'C', it needs to be sent to '1' and '3'.
Well. you guys get my drift. I guess I will be needing a table (therefor posting on this php forum). I am almost knowledgeless, so in detail would be most appreciated.
(code/scripts etc)....
And if you're gamer (PC/Xbox/PS3), I might even recruit you as a VIP member (if you want 😉 )

Are you asking for a starting point? Or an specific code/script that might be of use?

You'll probably be wanting a database to store the information of what Gamer likes what game and what their email address is etc. A good place to start might be:

That can be used closely in conjunction with php to produce your webpages.
