Can anyone show me how to do this form/submission page?

So on the form page I want to ask the user for
their name, street address, city, state, and zip code. With a select menu, I want to have
the user choose between a small, medium, or large t-shirt, small is $1.00,
medium is $1.50, and large is $2.00.
2) On the submission page I want to collect the submitted information and output the
user's name and complete address. Also I want to output the total
cost, which will include a sales tax of 5% and a shipping charge of $1.15 for small
and medium sizes or $1.50 for large.
Can anyone show me how to do this?
Would greatly appreciate it. Have been struggling for some time.

Which part(s) specifically do you need help with? What have you come up with so far?

do you want us to write the whole form for you?

Do you have a method of storage? xml, mysql etc, do you want the form to send an email to you and the user?

My first step would be to design/write the form in HTML

then create a form validator, loads out there for you to choose from.
