php form/submission page

So I am trying to build the attached page in PHP.
I am at a loss for how to proceed. I know I need to create a submit page also, but I don't know how to do that.

Any help you experts out there could offer me would be highly appreciated.
The radio buttons and checkboxes need to be outputted using PHP arrays and i'm not sure how to do this as well.

Hi and welcome,

Firstly I would recommend posting your code directly in this forum using the PHP tags that have been provided. It makes it easier for anyone trying to understand what you are trying to achieve.

From what I can gather you are calling this a "submit page", I think you are better referring to a a .php file with the processing code inside.

To be able to advise you better we would need to know what your objective is, for example; is this page submitting to a database, flat file or to email?

I would recommend taking a look at this article to give you a feel for what you are trying to do:

I'm so over my head. Big thanks to all responses so far.
this is what needs to happen:
1) Collect all submitted data in a logical, useable way.
2) Validate the submitted data. Required fields include the ticket "quality" and all of the name/address information. Amenities are not required.
3) Send the user back to the form if there are any errors. Output all error information and populate the form fields with the data previously entered - this includes radio buttons and check boxes.
4) If the submission is valid, calculates the user's total based on the ticket quality and amount of amenities ordered. Add a 5% surcharge and 6% sales tax.
5) Show the user all submitted data plus total(s) for confirmation.

Most people, including myself feel they are in over their head when starting out with PHP.

The approach I tend to make is firstly understand what I want to happen (The data model).

Second try to find examples on the net (Google Search) of open source projects or solutions others have created to help the community learn PHP.

It is very easy to use others code but I have fallen over many times because I didn't understand the basic principles of PHP.

My advice to you is this:

Search for examples of simple form processing. I already gave you an example in the post above. Once you have the basics you can build on that with your own design to fit your data model.

If and once you have tried to put something together and you are maybe having difficulty with some code, then post it in this forum and I am sure people will be glad to help you, as they have with me.

Hey folks. Thanks for all of your responses. I was able to make some progress.
I think my form field is pretty much done besides the CSS styling I need to do later to make it look pretty. The one bit of confusion I have in regards to the form field though is how to assign the money value to each array. How would I apply the $1.50 each to each option in the array? hope thats clear enough.
So now do I go ahead and create the submission field?

For me this is moving from a simple form process to an ecommerce cart solution. At the moment all you have shown us a a few arrays and the html code for the form that will post the values and the result is only "echo" printed out on the screen.

just print the value of whole array on the screen you haven't done any calculation what so ever you have just tried to print the value on screen after the post
