Help creating survey/questionniare form with login function

I've created a dynamic website using php, however I want to create a survey form where users can login and create surveys or view old ones.

I'm not sure where to start, if someone can help me towards the right direction.

suggest you google survey software PHP, and install it and muck around with it. Then when you have a specific problem or desired revision, post some code and let the folks here handle a bit-sized problem or feature. That's how this forum works, really. Give it a try!
