Form returns a value from a list

I'm trying to create an authentication form like the one in this page

The visitor must submit City, Country and Serial Number
the system checks the validity of the Serial Number through a list that my client
will give me in xls. form and if the serial number is in that list
it will return three alphanumeric values (shoe size, color and type of leather) from the same list.
I forgot to mention that the city and country must be recorded for statistical reasons.
Forgive me I'm newbie and I'm not aware of the terminology.

I forgot to mention that if the serial number is not in that list it will return a "Not valid serial Number" message.

It would be a lot easier to save the data in a database like MYSQL rather than reading from .xls
