Form and database

Hi,

I am totally new to PHP. I have html form which has 5 questions and answer. I would like to submit the question and answer to database once daily by entering the answer for the question.

NOTE: I also have username and password connected on PHP. so when user logins in he/she will get 5 questions with radio button options for the answers.

submitting them and adding to corresponding username and also the user should not be able to enter the data again for that particular day is kind of confusing

I know it is possible in PHP but I am not sure how to achieve it?

What I have achieved till now:

Username and password.
Once user logins in the user can see 5 questions and answers
user can choose the answers through radio button
user can submit the answers questions
question and answers are saved to database.
Not acchieved yet:

Submitting the form only once per day.

When saving the user's answers in the database, save the time and date there as well. When the user next logs in you can check that time and date and decide whether it has been at least a day since they last submitted the form.
