Email form and upload file to server

I have been working on a enquiry form for sometime now and i cannot get it to work. Basically i want to email the form contents and upload the files to my server.

Below is what i have created thus far but any hel would be greatly appreciated.

As for your issue, here are some questions/observations I have:

You never tell us what's wrong. What do you mean when you say you "cannot get it to work?" What happens? Do you get any error messages? Do you even have display_errors set to On and error_reporting set to E_ALL (or better)?
You never check to see if the file was uploaded successfully before you attempt to process it (or, even worse, tell the user that the operation was successful). See [man]features.file-upload.errors[/man] for more info.
You never check to see if [man]move_uploaded_file/man returned boolean FALSE (indicating an error has occurred).
Can you show us the HTML form that you're using to submit data to the above code?

Please firstly let me thank you for you response.

As i am sure you are aware this is something new to me which i am eagerly learning. The form displays all the correct messages to the user, however does not actually upload the file also i have been trying to find a way of uploading a file and emailing the form at the same time. Is this something that is possible to complete.

I am researching point 2,3 three to help me understand creating error.

While you're looking into points 2 and 3 above, you might also want to work on fixing your HTML markup so that it is valid (because, at present, there are numerous issues with it).

Of course! You can do any number of things you want to do with the submitted data... including send an e-mail.
