contact form with javascript and php

I was visiting the website "http://www.epipheo.com". There is a nice form (free_consultation_form) with form id "form_1" embedded into the home page. Its a php page. When we submit the form, It redirect us to a different page "http://www.epipheo.com/inquirysuccess/" and trigger a mail to our mail id.

I want to create a similar form, and hence I have copied the codes and edited it. For me, most of the feature of this website is working, except the contact form. I need help from the forum experts on how to replicate this form for my website. Please assist.

What issue specifically are you running into? We're not going to write the form for you.

thanks for your reply. If you see the contact form, it has a validation script. I want to create a similar contact form which can:
1. Do a client side validation
2. if the validation is true, it should redirect to the page: "http://www.epipheo.com/inquirysuccess" or similar
