Using PHP to create a new input box for each date in a range

Hi everyone

Just wondering if anyone is able to help?
I have a page for requesting holiday off work where someone inputs the start date of their holiday and an end date for their holiday.
I.e. 01/10/2013 to 10/10/2013.

Is anyone able to help with a simple script that would then based on that date range, output an input box for each date in that range? i.e. 01/10/2013, 02/10/2013 etc.
These would also have to then be dynamically named. And then how would I create a script that would loop through these and insert a new record into a table for each date?

Any help with be greatly appreciated as been struggling with this for months!
