search form for MYSQL database

Not really a newbie, but kind of am at the same time.
I need help creating a form which will go through my database and bring an output to the web page. For example several drop down boxes which have options of me retrieving a certain category of my database.
I've searching the web for days now for a good tutorial on how to do it but not much out there for it.
Would someone please help me out with this?

Can you give us specs? Database structure f.ex.? What you are asking for is pretty much a mini-app – so without any more specific info, it'll be hard to give you any info that's useful for you. Do you have anything set up for the task at hand yet? Database connection, etc.? Any trunk-code we can look at?
