please can anyone tell me how can i pass exec to php form.
that php form send email to the email account specified.

Not sure what you mean by "pass exec to php form"?

There is the PHP mail() function, which may be what you're looking for? (Not to mention php libraries such as PHPMailer and SwiftMailer that might be of use)
