Intermitment TNS-12154 Oracle8i

I'm getting an intermitment TNS-12154 when connecting through Apache from a browser. Has anyone seen this before? We've had this for two months now. The environment (Oracle_Home) is set at Apache startup and compile time, sqlplus connections from the client are OK and PHP can connect OK from the command line. It's only when we go through the browser (and the firewall) that the error is seen.

We've tried all combinations of Apache 1.3.9/Apache 1.3.12/PHP3/PHP4.

Do u have Oracle Client installed on ur web server ??
if yes,
have u configured, sqlnet connection, i.e tnsnames.ora file has a valid entry ??
have u checked ur sqlnet with
tnsping <servicename> ???
does, sql*plus works from Oracle client on web server ???
what is the third parameter u've given in OCILogon function ???
are $ORACLE_SID/$ORACLE_HOME configured ??
