I need to create a form.....

Hi:

My name is Fernando and I need to create a form to put it on my web-site.
I need to put the folowing fields:

Name of the visitor:-------------
Adresses of the visitor:----------
City of the visitor:---------------
Country of the visitor:-----------
E-mail of the visitor:-------------
Checkbox asking something:------
and
Coments and Suggests:----------

I dont have the form.php file, and I dont know how to create one of these.

Could somebody help me with this problem.....?
