It is not easy to develop and it is expensive to buy.

Oracle9i has Workflower facility
Documentum - wwww.documentum.com (do not know if it is possible to use data from DB or it is only for documents)
Another product http://www.wordware.com.au/
- you will hate it if you are Unix person (but may be you will steal an idea
Flower - www.pallas-athena.com
Free product:
www.arsdigita.com - you will need to learn tcl/tk or pay them for integration
I am looking for such product as well but it looks like there is no Open source solution here. Probably will decide to develop it with friends.

ANyway, what you said sounds like a piece of cake. You need a database (something like MySQL from www.mysql.com or PostgreSQL from www.postgresql.org), a database designer who can plan how your data will be stored, and a web server + apache + PHP. All freely available.
The application you described is fairly easy to develop. Thats pretty much all I can help, given my resource constraints.
