Does anyone know an example or tutorial of how to create a form that will upload a .pdf file to a web directory? Or would it be better to insert it into a Mysql database?

just a standard upload script can do this. If you want to create a odf in the fly like conveyorgroup.com/pdfr then it is a little more tricky, but can be done.
