If I have a web site like www.xyz.com and I want to add subdomains like
www.sub1.xyz.com www.sub2.xyz.com
What should I do? Do you I have to do this through the registrar (i.e. www.register.com) or through the ISP

This is pretty non-php related, but I'm guessing that this domain is hosted trough your ISP? If thats the case, then they can set it up for your, although they will probably charge you since it will require another IP address.
What you are trying to setup is called canoical domains (CNAMEs). Depending on who your ISP is there may or may not be a charge. My ISP does not charge for CNAMEs and even has a tech section where you can create them yourself.
Contact your ISP and ask them what steps you need to take to setup CNAMEs.
you can use a cgi script called forward pro to do that too. I don't remember the URL. I think it's go2script.com or something like that.

Not necessarily! There are many reasons to create another name, not all of which imply a separate host and/or ip address.
