Does anyone have any information or referrals for web hosting?

I thought I'd have a simple application but, as usual, things are getting more complicated. I need PHP4, MySQL, and someway to process credit cards so maybe SSL.

I'd rather not run my own server and was hoping someone had some recommendations.

One more requirement. I also need a way to generate PDF docs on the fly.

register for a cgi-bin account.
You'll get php and MYSQL here and you can choose wheter to display adds or not!
If you download mysqladmin and upload it (complete free mysql interface in PHP) you'll have a really neat place!
I have one too, so i know :-)

Give cihost.com a look-see, too. Like Craig, I just signed up there. Unix based or NT ... your choice. CGI / SSL and mostly free marketbasket thrown in for good measure. 2.5% override on CC sales. $40 a month buys a LOT of built-in stuff.
