Upload PHP Code

I am currenty upgrading my site in PHP but the one ting I am having a hard time with is if you have ever been to a site that allows you to upload files then you will understand this... I need to allow the user on my site to pick a file from there harddrive and then click a button to upload it to my system after filling out all the info...

Could someone help me out...?
