php Newbie, First Form does not Work...?

Hello, all. I have created my first form using a .php3 file as the action that will collect the information, put out a nice page saying thank you, and email the results of the form to two different users.

Here is the URL for the html page that has the form (http://www.kylabrescue.com/onlineapp.html) and here is the .php3 file that it should call up (http://www.kylabrescue.com/thanks.php3) It seems to call it up, but nothing happens and it eventually times out. I put the source code on the following file (http://www.kylabrescue.com/thanks.txt) so somebody could View Source, see the php coding, and correct me where I went wrong.

Have you tried using formmail.php? I found it extremely configurable and easy to use. It also has a redirection feature. Here is the link http://www.lumbroso.com/scripts/. It's called Jack's FormMail

don't be offended for me askeing this question, but is your server running PHP? If not, no matter how good your code is, it won't work...

also - I tried to view the pages you mentioned: the form looks very nice (you're very thorough), but I can't see the txt file.

 I am going to assume that php is running on your server, so, the phpinfo function should work. Try that as a test first to see if it is working. Just make a file with <? phpinfo(); ?> and save it in your document folder. Also, I don't know if you are using php3 and you have to track your variables from your form at the beginning of your php file.-->  <?php_track_vars?>. I can't see the form when I go to the page, neither can I see the text file. Another thing, I also see alot of printing with html tags but you are not printing to a browser because it is not in an html document. Hope these comments help.
