We have a MySql Database, and we are looking for an application that will allow us to conduct online surveys with the results posting into the database. Does anyone know of an application like this?

well, i would suggest rummaging around the code library here. look at sourceforge. do a search on google. or read the manual and build a quick script yourself. building a very simple survey is not too hard.
