Depends on what you're meaning when you say customizable. I've got one up for download on my site which you're more than welcome to if you like. The guestbook itself is relatively simple, but it uses an abstraction layer which supports FrontBase, MS SQL, ODBC, and MySQL so it's customizable that way🙂
