PHP upload

I'm trying to make a form using PHP to process the form. I want to have an upload option, but instead of having the file that the user wants to get to me be uploaded to the server, I want to have it emailed as an attachment. I also want the form to be easy to modify so I can add things such as name, address, email, etc. I have looked but haven't found this type of upload. Can someone help?

You can have a users email program activated when they hit a link but you can't have their program locate a file on their machine.

The option would be to have them upload the file to the server and then YOU do the emailing via a php script. Which kinda takes aways the purpose of having the upload in the first place doesn't it?
