Hello, im working on a script that takes information from a form and puts it to a web page. well i have it all written out and as soon as i think it is going to work i get this error that looks like this...

i am only using three files, not counting all the text files you see above, I'm using AddBasketball.php, which is the form that the coaches fill out, games.php which is he main part of the script, retrieves all the information just saved to the text files, then i have functions.php. If you can at all please help please reply or email me, if you want ot look over the files or view the page to help you get a better idea of what is going wrong please let me know.
