simple poll cookie

I'm trying to get a cookie to be set the first time the user takes a survey and if the user tries to take it again, it knows and redirects them elsewhere.

I tried many combinations without success~!

Can somebody take me gently by the hand and show me how to walk on the cookie road?

I am going to ask you a dumb question, don't mind. Is the browser that you are using set to accept cookies ?.

yeah I have tweaked the settings so that I can either accept them or refuse them just to test them. I have no problem accepting the cookies.
