Sample script?

Hello,

I don't know a lot of php so I generally use sample scripts from the internet. Now I want to setup a page on which you have to fill in your name, company and email address. After that the script has to check if everything is filled in and write the filled in information to a file. The user will now be re-directed to another page.

Does anyone knows a sample script for this or probbably can explain how to do it?
