Elusive error

I'm getting an "unexpected $" parse error on one of my form pages. PHP is telling me that it's on the last line of the page, but there's no PHP there.

I looked for the usual missing semicolon that throws "unexpected" errors, but can find none. Then I tore out chunks of code and kept testing, but I still can't find it.

If someone could help me out, check out this page (made it a txt file for ease of download) http://www.mixbartending.com/register_testing.txt

I too get a 404 error, but it might be worth mentioning that unexpected $ errors at the end of the page are often caused by forgetting to close all your braces, so have you checked that thoroughly? (you should just be able to do a count of how many { there are, and make sure it's equal to the } count)
