We, as php programmers, are able to gather all sorts of information from visitors to our sites. We can collect this information via forms- with the visitor's concent, or via code-ip address, and such.

Is there a good way to state this in a privacy statement?

Can you please point me to some resources?

Just be honest. Tell them exactly what information you're collecting, how long you're going to store it, and what you're going to do with it.
