Sending form data to a database

I'm trying to send data from 3 dropdown lists in a form to a database.

Anyone have any ideas about how to do this, and/or resources on where to find instructions?

I also have two of the lists connected by arrays and a php script which spits out database information, so I don't know if this makes it too complicated...
