I've sold webhosting for years, and find no one that offers all the things that you've requested. You'll get some offering only part of those, and others offering the rest.
You may want to check out prohosting.com.
I've only been with them for a short time, however, they are knowledgeable and very good at customer service. I switched to them because they offer UNIX with Apache, 200mb for $17.95 to start and up from there.
Be sure to check out there prices. I mean, $18/mo... that's practically nothing for what they give you!
