yes, in your subsequent page, you can have php insert the form variables into mysql easily -
howver, to avoid any headaches, I would use radio buttons rather than check boxes - eg either you smoke or you don't. At the moment I could select both, plus select drinking habits ranging from teatotaller to bingebasket :-)
However, the checkbox would probably work for "are you planning a pregnancy, or are you pregnant"
"NO, I wasn't, but now YES, I am"
