That is just a HTML page showing the layout of the PHP form i'm using.

I want to enter information into my MySQL database using that.

Can anyone tell me what code i need to put?

Will i need to put something like "INSERT INTO petition SET PetionName " and then the corresponding input-text box?

look for some of the excellent tutorials (here and elsewhere) dealing with forms and database access. They are suitable for people just learning.
