Form Editor Php/MySQL

I am seeking a WYSIWYG form editor that works for PHP/MySQL. The options must include a database/form/field selector for MySQL. Does somebody know of a program that can handle this (Windows/Linux)?

Only thing close I know of is JaneBuilder, but that is only for OS X
