Calculator and MySQL

Hiya does any one know where i can get a script in php that uses multiple values from a table in a db and calculate them against a value inserted via a form?

ok in my db i have 3 rows a b c each of these rows have 6 colums with figures in them what i need to calculate is the user selects 1 option in the form (either a, b or c ) then then enters a dollar amount i need that dollar amount they entered to be calculated against which ever row they selected when they chose a, b or c
