I found the lizquiz script on this site and the page states:

I could not reach this site.
I'm very anxious to get the script working. I provide free Windows 2000 and cisco exams at http://www.certyourself.com and I've been searching for months now to find a quiz script that shows a question per page and can have multiple correct answers (I hope through checkboxes, so that they must choose all the correct answers..??) I have really tryed 20+ different scripts....
