dynamic drop down menu

I wish to make a drop down menu that allow user to select from the drop down list, OR key in their own data if its not availabe in the drop down list.

How do I do that?
Can someone give me the code?
Any help would be greatly appreciated as I need it quite urgently.

The div's are at the bottom of the screen
the table about 60% way of the source.
This is the same drop down as microsoft.com uses (beware, doesn't support Netscape)
